Description of MyChart

Children's Hospital Colorado & participating PedsConnect Pediatric Practices are pleased to offer our patients access to portions of their medical records via an Internet-based version of our electronic medical record called MyChart. This service includes information related to a patient's current medications and appointments, and permits patients to submit questions to Children's Hospital Colorado and PCPP health care providers online.
MyChart is a secure Internet portal that allows current Children's Hospital Colorado and PCPP patients and/or their parents and legal guardians to view select areas of their electronic medical records. This is a courtesy service that Children's Hospital Colorado and PCPP makes available to families as they visit our facilities for their ongoing medical needs and is intended for non-urgent communication only.

We think it's important for you to know how we handle information communicated online. This Terms & Conditions statement outlines our practices and our sensitivity to your right to privacy. It also outlines our expectations about how MyChart will be used by the families who enroll and accept these Terms & Conditions.

Limitations on Use

MyChart should never be used in place of calling 911 or calling your child's health care provider in the event of an emergency. Please note the medical or health care information provided via MyChart is not a replacement for proper medical attention. The portions of a patient's electronic record visible through MyChart are the property of Children's Hospital Colorado and PCPP and do not represent the patient's complete medical record. Medical information available via the MyChart service is solely intended for the patient and/or patient's legal guardian(s) and is not intended as a method to transfer or provide medical records to other medical facilities or medical providers. Any requests for copies of medical records for the purpose of continuity of care should be directed to the Health Information Management Department of Children's Hospital Colorado and PCPP where the patient has received treatment.

Response to Electronic Communication

If a patient or parent submits a question to a Children's Hospital Colorado and PCPP provider via MyChart, the appropriate Children's Hospital Colorado and PCPP staff will use their best efforts to provide a timely response within approximately 2 business days. MyChart users should not submit questions related to an urgent medical situation via MyChart; rather they should make a phone call to their child's health care provider or visit an emergency room.

Children's Hospital Colorado and PCPP staff will respond to such communications based solely on the information provided by the patient or their legal guardian. If there is insufficient information provided, we will be unable to provide an appropriate response. Children's Hospital Colorado and PCPP will assume that all communication through MyChart is in regard to the enrolled patient's health condition. Asking for health care advice on behalf of another person could be potentially harmful. In addition, we cannot provide medical advice if you have not previously been seen by the provider. All communications through MyChart will be saved as a permanent part of the patient's medical record. Children's Hospital Colorado and PCPP does not assume any responsibility for health information or services used by persons other than the MyChart user.

Email Privacy

Information sent via MyChart may be shared with Children's Hospital Colorado and PCPP staff assisting the health care provider in providing the child's medical care. MyChart users will be notified via email when there is new medical information available regarding the patient within MyChart. This means that any person with access to the contact email provided on the MyChart application will be able to see this notification. This could include spouses, employers, or anyone else with access to that email account. Although no private medical information will be sent via email, the notification that new medical information is available by accessing MyChart may be information you would not want others to know. You should take this into account when providing an email address.

Security & Confidentiality

Children's Hospital Colorado and PCPP  adheres to all applicable federal and state laws regarding the confidentiality of patients' medical records, including the Health Insurance Portability and Accountability Act (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH). We afford the same degree of confidentiality to medical information stored in our electronic medical record as is given to medical information stored in any other medium. Children's Hospital Colorado and PCPP are committed to protecting the confidentiality of your medical information. Firewalls, passwords, encryption, and audit trails are further used to safeguard patient information. Children's Hospital Colorado and PCPP maintains a log including the time and date of access for each record that is accessed through MyChart.

If Children's Hospital Colorado and PCPP  suspects or confirms that an enrollee has inappropriately shared their password with another person, or that the system privileges have been misused or abused in any way, that patient's access to MyChart will be discontinued without prior notice. Children's Hospital Colorado and PCPP will take appropriate legal steps to remedy such a confirmed security breach. Children's Hospital Colorado and PCPP are not responsible or liable for disclosures of medical information obtained through unauthorized use of a MyChart user's permanent password or the sharing of this information with individuals not legally authorized to view a patient's medical information.

As a MyChart user, your role in maintaining the security of your medical information is: 1) changing your password on a regular basis, 2) keeping your login ID and password confidential, and 3) accessing MyChart only from computers that are believed to be secure; you should not access MyChart from any public, shared computer such as those found in public libraries or Internet cafes since they may be compromised.

Children's Hospital Colorado and PCPP has taken steps to make all information received from enrolled MyChart users secure against unauthorized access and use. For other than general information viewing, MyChart must be accessed with a browser that supports Secure Sockets Layer (SSL). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available.

MyChart Availability

MyChart may not be available to you at all times due to system failures, procedures, maintenance, or other causes beyond the control of Children's and PCPP. Access is provided on an "as-is, as-available" basis. Children's Hospital Colorado and PCPP cannot guarantee that users will be able to access MyChart at all times. If MyChart is unavailable, parents should call their child's health care provider with any questions they may have or to obtain information about their child's medical care or condition.

Parents/Legal Guardian Access

Parents or legal guardians may request access to their child's MyChart account by requesting proxy access. Patients older than 14 and up to age 18 may have the legal right to consent to certain types of medical treatment themselves, without the necessity of parental or guardian consent. While parents and legal guardians will always have the right to request copies of complete medical records for their children as permitted by law, Children's Hospital Colorado and PCPP respects the rights of these young patients to confidentiality regarding their medical care.

Agreement to Terms and Conditions

By clicking the "Accept" button below, you agree to the statements and terms contained herein. Children's Hospital Colorado and PCPP reserves the right to revoke a MyChart user's access at any time for any reason. You can choose to dis-enroll from your use of MyChart at anytime by calling 720-777-4357 or send an email to Children's Hospital Colorado and PCPP may modify this Terms and Conditions Statement at any time.
